Sanford Health is pleased to offer you member information via a Web-based version of our computer systems called My Sanford Chart.

To request an account, submit a My Sanford Chart Consent Form to your clinic, indicating you have read this Terms & Conditions Statement.

We think it is important for you to know how we handle information we communicate via the Internet. This Terms & Conditions statement outlines our practices and our sensitivity to your right to privacy. We reserve the right to revoke access at any time for any reason.

Response to Electronic Communication - Sanford Health will make its best effort to provide a timely response to electronic inquiries. In some cases, the staff that needs to respond to an electronic inquiry or other communication may not be immediately available so users should allow at least two (2) business days for a response. Accordingly, emergency situations requiring immediate attention should not be submitted electronically. My Sanford Chart should only be used to request advice for non-urgent situations. If you have an urgent need to see a medical provider, please contact your clinic by phone. For a medical emergency, please call 911 immediately.

Furthermore, with respect to any electronic communications, we are only able to respond to such communications based on the information provided by the user. If there is insufficient information provided, we will be unable to provide accurate and reliable services.

Web Site Links - My Sanford Chart may offer links to related websites not owned or managed by Sanford Health. These website link(s) are for informational purposes only. Sanford Health does not endorse and has not verified the accuracy of the information in/on these websites, and the user should not rely on any of the information found on the websites for purposes of treatment or diagnosis.

Minor Eligibility - An individual must be age 12 or older to request a personal account. Parents or guardians of minor children may request access to the minor child's account by following the terms outlined in the "Proxy Access" section of this Terms and Conditions statement. Access may be revoked when confidential care has been provided, parental rights have been restricted, or when required by law.

Proxy Access - Individuals age 18 or older may request proxy access to another individual's My Sanford Chart account by completing the Proxy Access portion of the My Sanford Chart Consent Form and submitting it to Sanford Health. Proxy access to individuals under the age of 18 will only be granted to parties with parental rights or permanent legal guardianship over the individual. Parents or permanent legal guardians may be required to produce documentation demonstrating their relationship to the individual. A My Sanford Chart account will be activated for both the proxy and the patient. Granting proxy access allows the proxy to view and interact with all information in that individual's My Sanford Chart, including information added through current and future medical record sharing platforms, health information exchanges, and from other non-Sanford Health records that may have been linked to that user or individual within My Sanford Chart or through a third-party application. Proxies may also receive email notifications, such as those related to the individual's medical or billing information contained in that individual's My Sanford Chart. If the proxy's permanent legal relationship with the individual changes, the proxy must inform the Sanford Health immediately by calling or sending written notice. Sanford Health reserves the right to revoke proxy access at any time or for any reason.

Email Privacy - My Sanford Chart users should be aware that they will be notified via email when there is new information to be viewed in My Sanford Chart. Proxies may also receive copies of any emails received from My Sanford Chart. This means that any person with access to an individual's email or their proxy's email accounts will be able to see this notification. This could include the user's spouse, employer, or anyone else that can access a user's email account. Although no private medical or insurance information will be sent, the notification that new information is available by accessing My Sanford Chart may be information that a user would not want others to know. Thus, users should take this into account when providing an email address.

Please know that if you send us an email communication, it may be shared with appropriate Sanford Health staff to assist in answering the inquiry or concern.

Security and Confidentiality - We afford the same degree of confidentiality to medical and insurance information stored in My Sanford Chart as is given to information stored by Sanford Health in any other medium. Sanford Health is committed to protecting the confidentiality of your medical and insurance information. We limit Sanford Health employees' access and ability to enter or view information based upon their role. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. We shall identify the records released and note the time and date of access each time a user accesses My Sanford Chart. We have taken steps to make your information as secure as possible against unauthorized access and use.

For other than general information viewing, My Sanford Chart must be accessed with a Transport Layer Security (TLS) modern browser. Our TLS web server uses authentication and offers the highest level of encryption technology commercially available (256-bit AES).

You can tell when you are secure by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server. This means your data cannot be read or deciphered by unauthorized individuals. You can tell whether you are truly connected to Sanford Health by viewing the digital certificate. This certificate verifies the connection between the Sanford Health server's public key and the server's identification.

User names and passwords provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet. As a My Sanford Chart user, your role in maintaining the security of your medical information is: 1) Changing your password on a regular basis, and 2) Keeping your login ID and password confidential.

Third Party Services - You may choose to use other services provided by third parties in conjunction with your use of My Sanford Chart. Such services may include, but are not limited to, translation services, location services, and other third-party offerings. Any links to such services are provided for your convenience only. Sanford Health has no control over the contents of these services. Sanford Health does not accept any responsibility for these services or any loss or damage that may arise from your use of these services. Such services may require you to submit your information to a third party, which may result in a copy of your information being used and/or stored by a third party. If you decide to access any such services in conjunction with your use of My Sanford Chart, you do so entirely at your own risk and subject to the terms and conditions of use for such services.

Video Visits - Sanford Video Visits are available for any non-emergent condition for a user via the user's own My Sanford Chart account or by a person with valid proxy access to the patient's My Sanford Chart account. If a Sanford Video Visit is submitted by a proxy for a patient under the age of 18, the proxy must be the parent or permanent legal guardian for that patient. Be sure you are in the correct medical record when submitting a video visit request. The patient must be present to participate in the video visit. Except as set forth above, you may not use your My Sanford Chart account to seek a Sanford Video Visit for another patient.

Proxy access allows a person to access information in another person's medical record. Typically, parents will use this to access their children's accounts. Adults may also use it to access a spouse, older parent or relative's record. To request proxy access to a family member, click here. As stated above, granting proxy access will also allow the proxy to view and interact with all information in that individual's My Sanford Chart, including current and future medical record sharing platforms, health information exchanges, and third party applications which may allow the proxy user to view non-Sanford records that have been linked to that individual's My Sanford Chart. Additionally, the proxy may receive email notifications, such as those related to the individual's medical or billing information contained in that individual's My Sanford Chart.

Computer Requirements - A Sanford Video Visit can be completed on a desktop or laptop computer with a webcam, microphone, and stable internet connection. Supported browsers are Microsoft Edge, Google Chrome, Apple Safari and Mozilla Firefox. A Sanford video visit can also be completed on an Android or Apple mobile device or tablet via Sanford Health App. Data rates may apply to your use of My Sanford Chart, including video visits. Please contact your internet service and/or mobile device provider to determine applicable rates.

Appropriate Use - By requesting a Sanford Video Visit, you agree to abide by the standards of conduct appropriate for an in-person clinic visit. Explicit, threatening, or abusive behavior or communication or other activities which are not compatible with appropriate use of Sanford Video Visits will not be tolerated and may warrant termination of a Sanford Video Visit.

Privacy Assurance - Your Sanford provider considers your Sanford Video Visit to be a private patient encounter. By accepting these Terms and Conditions of Use, you acknowledge and accept responsibility for any third party in or around your location who observes or hears your Sanford Video Visit or who accesses your computer or mobile device in any fashion.

Photographs - Please note that depending on your complaint or condition, it may be necessary for your provider to take a photograph during your Sanford Video Visit. By accepting these Terms and Conditions of Use, you consent to such photography if appropriate for your care.
